一、项目概况 1、项目名称：分中心消防改造项目 2、工程概况：分中心消防改造项目包括应急照明、自动报警、喷淋系统和2路供水。 3、项目总投资：门诊手术室改造60万，工程造价咨询费限价0.17万元。 二、服务要求 1、服务期：合同签订之日起至项目审价工作完成。 2、服务内容：承担本项目的工程竣工结算审价工作。 三、供应商要求 1、具备工程审价或相关营业范围的独立法人或其他组织。 2、具有工程造价咨询资质及相应人员配置。 3、未被列入失信被执行人、重大税收违法案件当事人、政府采购严重违法失信行为记录名单；一年内没有工程造价咨询规定的禁止行为，一年内没有因本企业造价咨询责任造成重大事故。 4、竣工结算送审后一个月内完成审价工作并出具审价报告。 5、本项目选派方式为竟价选取

（一）在中介集市报名前3个月内存在1次记入一般不良诚信档案行为的；

（二）在中介集市报名前1年内存在2次记入一般不良诚信档案行为的；

（三）在中介集市报名前2年内存在1次记入严重不良诚信档案行为的；

（四）在中介集市报名后中介服务合同签订前，存在记入一般或者严重不良诚信档案行为的

I. One recorded minor bad credit behavior within 3 months before registration in the intermediary market.

II. Two recorded minor bad credit behaviors within 1 year before registration in the intermediary market.

III. One recorded serious bad credit behavior within 2 years before registration in the intermediary market.

IV. Any recorded general or serious bad credit behavior between the registration in the intermediary market and before the signing of the intermediary service contract.

回避情形

（一）参与同一项目采购的不同中介服务机构法定代表人或实际控制人为同一人的；

（二）中介服务机构的法定代表人或实际控制人和委托人单位法定代表人或者项目负责人有配偶、直系血亲（含拟制血亲）、三代以内旁系血亲或者近姻亲关系的；
